Client Portal I Forms

 As part of our commitment in delivering the best care and service to you, Palmetto Counseling recently upgraded our electronic record system. We are pleased to offer a HIPAA compliant and secure Client Portal in which you can fill out and complete new client paperwork, upload documents, update your information, or securely communicate with your therapist. In addition, you now have the option of receiving electronic text message, email, or telephone appointment reminders.

Please remember to bring your insurance card(s), any required insurance or EAP referral forms, all applicable co-pays, and at least (x1) form of valid picture identification / drivers license with you. 

Important Note: To save time during your first appointment and allow you more time with your therapist, please choose one of the following options to complete new client paperwork and expedite client registration process 


 1). Client Portal - PCC Intake Forms

 To access PCC's Client Portal, you will need to call our Office at (803) 329-9639 to receive your assigned User Name and Password.

 Click on 'Client Portal'

 Enter your assigned User Name and Password and click 'Log In'

 To access and complete PCC Intake Forms - Go to top menu and click on 'Docs'

On the first row, you will see PCC Intake Forms....look to the right hand side and click 'View'

After double-clicking on 'View' you will see PCC Intake Forms are ready to download as a 'Word' document [typically found on bottom, left-hand of your screen. If unable to view the forms on your screen, go to your browser and click 'Downloads'] Note: In some cases you may see a yellow warning banner alerting you document is in protected view. Click on 'Enable Editing' to allow you to write and save your document.

Once downloaded, complete the forms by typing in all requested fields with your information. After you've finished, go to 'File' and select 'Save As' and name the document and location you would like to save your file.

IMPORTANT: Once saved, scroll down to the bottom of the 'Docs' screen and find 'Upload files to the Practice' and click on 'Select Files' [select saved file] to upload it to Client Portal. 


 2). Client Portal - Client Health Questionnaire 

 To access PCC's Client Portal, you will need to call our Office at (803) 329-9639 to receive your assigned User Name and Password.

 Click on 'ClienPortal'

 Enter your assigned User Name and Password and click 'Log In'

 To access and complete PCC Client Health Questionnaire  - Go to top menu and click on 'Forms'

Using Pull Down Menu, click on 'Select Form to Complete' and click on 'PCC Client Health Questionnaire'

Complete and fill in all required fields. 

IMPORTANT: Once complete, scroll down to bottom of screen and click 'Save PCC Client Health Questionnaire'


3). Download and Print Hardcopy Versions of PCC Intake Forms and Client Health Questionnaire by clicking on links below.

Please download, print and complete all forms and bring them with you to your first appointment. 

4). Please have all forms completed for your appointment or plan to arrive at least 30 minutes early to complete them in the office.
